3. Eligibility

By using our services, you confirm that you meet these requirements.
4. Description of Services
H4M provides access to:

- Hormone optimization programs.
- Prescription therapies (when clinically appropriate).
- Lab testing services (including third-party labs).
- Nutritional supplements (including DIO™).
- Wellness and lifestyle optimization services.
Medical services are provided by independent, licensed healthcare providers and are not provided directly by Optimal Health Management, LLC.

8. Payments & Billing
All services are self-pay, unless explicitly stated otherwise.

- Pricing is disclosed prior to purchase.
- Payments are processed via third-party providers (e.g., Authorize.net, Stripe).
- You authorize all charges for services, products, labs, and prescriptions.
Memberships & Recurring Billing: If you enroll in a recurring plan:
- You will be billed automatically on a recurring basis.
- You may cancel according to applicable terms.
- Charges already processed are non-refundable.
9. Refund Policy
Due to the nature of the services:
- Medical services are non-refundable.
- Lab testing and prescriptions are non-refundable once ordered.
- Supplements may be eligible for return at our discretion.
10. Prescription Policy
Prescriptions are:
- Issued solely at the discretion of licensed healthcare providers.
- Based on medical necessity and appropriateness.
- Fulfilled through licensed 503A compounding pharmacies.
We do not guarantee eligibility for any treatment.
11. Third-Party Services
We work with third-party providers, including:
- Laboratories.
- Pharmacies.
- Payment processors.
- Technology platforms.
We are not responsible for delays, errors, or outcomes caused by third parties.
12. Privacy & Data Use

20. Financial Terms & Payment Policies Addendum
By checking the agreement box, you acknowledge that you have reviewed and understand the following financial terms and payment policies.
21. Commitment to No Auto-Billing
- H4M does not operate on a subscription model.
- H4M will never automatically charge your saved card.
- H4M will contact you for confirmation before any invoice or charge is processed.
- There is no auto-renewal and no auto-billing.
- Each plan period is a separate transaction and continuation is always your choice.
22. Financial Policies and Plan Payment Structure
H4M manages billing for Precision Protocol services provided by licensed providers who do not accept insurance. As a self-pay practice, H4M does not process insurance reimbursements.
- You are responsible for all payments, including services, laboratory tests, supplements, and additional prescription fees.
- No protocol services, including prescription orders to the compounding pharmacy, will be processed until payment is received.
- All plans are paid upfront for the selected period. Any monthly pricing shown is for comparison purposes only.
- Accepted payment methods include credit card, debit card, ACH, check, or another approved method.
23. Plan Continuation, Refills, and Shipping
- As your plan period approaches completion, your Health Maverick will notify you that your refill window is open.
- Continuing is optional and there is no obligation to renew.
- You may switch plans at renewal.
- Prescription orders will not be placed until payment for the new period is received.
- If you choose not to continue, your patient file remains active and you can restart without repeating onboarding.
- Shipping typically takes 4-7 business days from pharmacy order.
24. Additional Fees and Services
- Custom plan patients are responsible for lab costs at the time of service. Vitality and APEX plan lab inclusion follows plan descriptions.
- DIO™ costs are separate from plan pricing. Published member discount terms apply by plan.
- Individual prescriptions requested outside your standard protocol incur a $60 transaction fee per prescription, with notice before any charge.
- H4M can provide a superbill with billing codes for independent insurance reimbursement attempts.
- Outstanding balances must be cleared before further services.
25. Refunds, Cancellation, and Service Modifications
- Medications are non-refundable once a prescription order has been placed with the compounding pharmacy.
- If payment is made but the prescription has not yet been ordered, a refund may be issued upon request.
- You may discontinue at the end of any plan period with 5 business days' written notice.
- No refunds are provided for services already rendered within the current period.
- H4M may modify protocol services with 10 business days' written notice.
26. Peptide and Supplement Disclosure
H4M coordinates both prescription peptides (through licensed providers and licensed 503A pharmacies) and research peptides.
- Research peptides are not FDA-regulated or approved, and by electing to receive them, you acknowledge that distinction.
- H4M coordinates sourcing and fulfillment but does not manufacture, prescribe, or guarantee efficacy of research peptides.
- These products may carry risks, including medication interactions. Report adverse effects to your provider immediately and discontinue use until advised.
- Special orders require prepayment and are non-refundable and non-exchangeable.
- Payment must be received before orders are placed.
- Supplements coordinated by H4M are not FDA-evaluated and are not intended to diagnose, treat, cure, or prevent disease.
