This Privacy Policy describes how OPTIMAL HEALTH MANAGEMENT LLC DBA H4M ("H4M," "we," "our," or "us") collects, uses, shares, and protects your information when you interact with our website, telehealth platform, app(s), or services (collectively, the "Services"). We are a 100% telehealth provider offering hormone therapy and related consultations on a self-pay basis - no insurance is involved.
"Personal Information" means any information linked or reasonably linkable to an identified or identifiable individual. "Protected Health Information" (PHI) refers to health-related information subject to HIPAA.

We need certain information to provide telehealth consultations and services; without it, we may not proceed.
We may combine data and create de-identified/aggregated data for any purpose (e.g., research, marketing).
We use tools like Google Analytics. We may share hashed identifiers for targeted ads (may qualify as "sale"/"sharing" under some laws - opt-out available). Opt-outs: Google Ads Settings, Facebook/TikTok preferences, or email us. We do not respond to Do Not Track but honor Global Privacy Control (GPC) where required.
We do not sell personal information except as described in advertising opt-outs.
Opt-out of marketing emails via link or contact us. For sensitive data (health info, gender identity, sexual orientation, hormone treatments), we limit use to providing services unless you consent/opt-in for other purposes (e.g., ads). Opt-out/limit via email or GPC.
Services not for under 16. We delete any such data if discovered.
We retain as needed for care, legal compliance (e.g., medical records per state laws), then securely delete.
We may update; material changes posted with notice. Continued use = acceptance.
We comply with HIPAA for PHI. This NPP explains uses/disclosures of PHI (treatment, payment, operations) and your rights (access, amend, restrict, accounting).
For substance use disorder (SUD) records under 42 CFR Part 2 (if applicable/received): Stricter rules apply - uses/disclosures generally require written consent; limits on redisclosure; heightened protections (e.g., no use in proceedings without order/consent). We align with HIPAA where possible but follow more stringent Part 2 limits.
Transmission We use encryption (SSL/TLS) for PHI transmission.
Third Parties Not responsible for linked providers/pharmacies - review their policies.
Technical/organizational measures (encryption, access controls). No 100% guarantee; you protect credentials.
CCPA/CPRA rights: Categories collected/disclosed (identifiers, health data, etc. - see chart in original). Opt-out of sale/sharing. Email support@hormonesforme.com. No discrimination.
(CO, CT, DE, FL, IA, IN, KY, MD, MN, MT, NE, NH, NJ, OR, RI, TN, TX, UT, VA) Rights include: Confirm processing/access, correct, delete, portability, opt-out of targeted ads/sale/profiling (some states require opt-in for sensitive data). Respond within 45 days. Email to exercise.
